Moving
Moving into or out of The Anchorage area or Alaska itself can have an effect on your Welfare Plan benefits if you need to obtain medical services. In any case you need to complete a Change of Address form and submit it to the Plan Office.

PPO Plan Networks vary depending on whether you live in the Anchorage area, elsewhere in Alaska or out of state. In particular, there is an additional, separate $1,000 hospital deductible if a PPO Provider is available in your area and you do not use that provider. So check the network availability for your area when you move.

It is your responsibility to notify the Trust Customer Service Office when you move. If you fail to do so, you may not receive important communications regarding your benefits.
