a.

5 years of Contributory Years of Service; and

b.

1,500 Hours of Covered Employment (excluding Recaptured Hours) in the 36 months prior to disability; and

c.

Total and Permanent Disability

 

Basic Benefit

Accrued Monthly Normal Retirement Benefit actuarially reduced from age 65.
Reduction Factor Table 1

Minimum Benefit: $69.90 times Contributory Benefit Service up to $489.30, but not to exceed the Accrued Monthly Normal Retirement Benefit.

 

Forms of Payment

a.

Straight Life Annuity
(Only form available if unmarried)

b.

Five-Year Certain Life Annuity

c.

Joint Annuity

Reduction Factors
All forms of payment except the Straight Life Annuity are actuarially reduced.

 

Conditions Which May Result In Loss of Some or All Benefits

Recovery from Total and Permanent Disability
