Life Insurance Benefit	Participant - $25,000 Spouse and Dependent Child - $2,000
Accidental Death & Dismemberment	In addition to the amount payable for life insurance, for the participant only an additional benefit is available if the death or a dismemberment is caused by an accident. The full AD&D benefit of $5,000 is paid for the loss of life or for any two: hands, feet, eyes, or combination. Half the benefit ($2,500) is paid for the loss of any single hand, foot, or eye. The maximum payable amount is $5,000.
