Eligibility

The Plan provides Dental benefits only to those working participants considered "Eligible Employees" including their eligible dependents and eligible Retired Participants and their dependents age 65 and over.

 

Your Treatment Options

In general, benefits are limited to the least costly treatment which is generally considered appropriate by the dental profession. If you choose more costly treatments, you are responsible for the difference in cost. Your treatment choices are up to you and your dentist.

 

Dental Calendar Year Deductible

$75 per Participant
waived for Orthodontics

 

Dental Calendar Year Maximum Benefit

$2,000 per Participant

 

Class I - Diagnostic and Preventive: Exams, x-rays, fluoride, cleanings, sealants.

80%

 

Class II - Basic Dental:Fillings, root canals, periodontal work, extractions, anesthesia, temporary crowns.

80%

 

Class III - Major Dental: Crowns dentures, bridgework.

50%

 

Orthodontics - Dependent children under age 19

50%; up to $1,200 lifetime;
$75 deductible waived

 

Treatment Plan

Have your dentist submit a treatment plan and request a pre-treatment estimate prior to beginning work which will total more than $500. This way you will be sure of what the Plan will cover before you receive treatment.
