1.6 REINSTATEMENT OF ELIGIBILITY FOR EMPLOYEE

If an Employee’s eligibility terminates due to insufficient hours credited or banked, the Employee can reinstate eligibility in one of the following ways:

  1. Within 13 months after coverage ends, the Employee is credited with at least 160 hours in two consecutive months. Eligibility is reinstated on the first day of the second calendar month following the month in which the Employee completed the 160 hours.

    Example: Coverage ends on January 31. If the Employee works 40 hours in March and 140 hours in April, eligibility for coverage begins again on June 1st.
  2. If more than 13 months elapse after coverage ends, the Employee again meets the initial eligibility requirements described above in Section 1.1.
