LIFE INSURANCE AND ACCIDENTAL DEATH & DISMEMBERMENT BENEFITS PROVIDED BY
STANDARD INSURANCE COMPANY
Life Insurance and Accidental Death & Dismemberment Benefits are insured by the Plan’s Life Insurance Provider. These benefits are described in detail in a separate booklet. For a copy of this booklet, call the Trust Customer Service Office. These benefits are also subject to insurance policy limitations, which are on file in the Trust Customer Service Office. In the event of ambiguity between that policy and the booklet, the policy shall control.
